In the care for people with a mild intellectual disability, small signals often play a major role. Recognising changes early requires attention, experience and—most importantly—clear tools. That’s why Stichting Altrecht Zon Mw and Utrecht University of Applied Sciences, together with Dex, developed a toolbox full of theme cards, manuals and a clinical signalling plan. A practical tool that supports care professionals in identifying issues sooner, aligning more effectively and taking more targeted action. It brings clarity and calm, especially in situations where quick decision-making is essential.
From content to a clear, recognisable toolbox
Together with subject-matter experts, we created a structure for the toolbox that feels intuitive for care teams. We developed a visual design and layout that make the information easy to understand and pleasant to work with: friendly, calm and directly applicable in practice.
In addition to a printed version for use on the wards, we also produced physical sets that teams can easily share and use during meetings. We also created digital versions that align well with training and e-learning modules, ensuring the toolbox is accessible in multiple ways and in different settings.
Impact visible in practice
The toolbox is now widely used and helps care professionals speak the same language more quickly. Teams recognise signals earlier, coordinate more easily and act with greater confidence. The result is a care process that runs more smoothly and predictably, with a direct impact on patient well-being. As one of the coordinating nurses put it: “The toolbox is clear, practical and helps our team align more quickly and effectively.”
